What is the best protocol for granuloma treatment post Volbella filler?

I received 1.5mL of Vobella injected into my lips. 5 months later I noticed a lump in my lower lip.Within a month time I had several nodules on both my upper and lower lips with my lower lip feeling firm to the touch. I was given a Medrol dose pack. The symptoms lessened while on the pack but soon returned after completion. I was informed the next line of treatment is 5fu+Kenalog+lidocaine. Is this the appropriate next step? What are the side effects? Do Granulomas resolve on their own with time

I recommend following up with a cosmetic dermatologist.  Granulomas need either kenalog and/or 5 FU. I do 5f/steroids for all my patients with filler complications. Best, Dr. Emer

Nodules after Voluma, Volbella and Vollure have been reported. These reactions have been reported months after injection and sometimes have been associated with recent dental work. Some people have felt that they are related to the development of a biofilm in the area of injection. They generally are best treated with a course of antibiotics such as doxycycline. They generally take 6-8 weeks to resolve. Dissolving this type of hyaluronic acid can be challenging. I would suggest following up with your provider and considering a course of doxycycline 150 to 200 mg a day for 6-8 weeks.

Filler granulomas can happen with any filler, unfortunately. 1) Sometimes they do resolve on their own - however they don't always. And if they do, that can take months. 2) Was hyaluronidase injected into the nodules? That would be my first step before even the Medrol dose pack. Hyaluronidase is the "filler dissolver" that can break up the filler causing the granuloma or nodule. 3) IL- 5FU/Kenalog/Lido is a good option, but only in patients that have failed aggressive hyaluronidase attempts. Risks with that combo of Kenalog/5FU is atrophy caused by the steroid.
