I would start with very dilute kenalog injections and see how the keloid responds. The key here is to use the weakest dose of kenalog that does the trick. You can create problems and deformities with steroids if too strong a dose is used or too much is placed. You may also want to consider silicone sheeting to help with the problem......and NO MORE PIERCINGS!
One of the best treatments for keloids and hypertrophic scars is injections with a combination of Kenalog and 5-fluorouracil into the scar. A series of injections can be very successful at flattening and shrinking the scar. If the scar is very small a surgical removal can be considered but injections should be done afterward to prevent recurrence of the scar.
