Help. Rhinoplasty 16 months ago, had multiple sinus infections since the surgery. The tip of my nose has puss coming out!

Since my Rhinoplasty 16 months ago iv had continuous sinus infections, with a foul odour inside my nose. The tip of my nose has been getting harder and harder over time and is now enlarged red In appearance and extremely painful to touch. My GP and the ER are hopeless and keep sending me home, so I applied tropical numbing cream, sterilised it and poked a 27g needle into the very top of my nose and a lot of puss came rushing out! What could be the problem here and what should I do?
