I had an upper blepharoplasty 9 months ago, I’m not seeing much difference from how my eyes looked before the procedure. My surgeon has offered to do a revision and take more skin from the outer edges of both my eyelids. Will this give a more defined crease and make my eyelids look smoother, I am also concerned about my eye shape being changed after the revision.
Answer: A revision should help benefit you. Thank you for your photographs. It does appear that you have residual excess skin of your upper eyelids. The contour and shape of your upper eyelids as well as the definition of your upper eyelid crease should be improved by a small revision. As always, the parameters of the revision should leave your central upper eyelid crease approximately 1 cm above your eyelashes and the lateral incision about 5 mm above the lateral canthus. In addition, the removal of the excess skin should only go up to the junction of the thin upper eyelid skin and the thicker skin which is just below your eyebrow. Properly done, this should not change your eye shape, but should give you a better, more rejuvenated appearance to your upper eyelids. Best wishes.

Answer: Eyelids it does look like you could benefit from a little more skin excision esp on the left eye, although I would caution you and your surgeon to be conservative because since it'll be a revision, this time aroun d you may develop some more scarring, which may then result in issues closing your eyes, among other things. you also seem to have deeper set eyes, which puts you at a slightly higher risk for that as well.

I think there is certainly some room for improvement in the amount of excess skin of your upper eyelids (more on your left than the right ). But the big question is whether this is just excess eyelid skin or if it is because your eyebrows are a bit lower which contributes to the weight of the upper lid and can be a reason for a failed initial blepharoplasty. I would discuss this with your surgeon and make sure they are taking this into account as I think this is the most common reason why patients need revision upper blepharoplasty.
