Need advice regarding a legal document my surgeon is requiring me to sign in order to correct his Revision Rhinoplasty surgery (my first). At 15-months post-surgery, he suggested further surgery to correct the results, but I couldn't travel due to pandemic. At 3 years (now), he no longer wants to do it, although will honor previous word, but only if I sign this document (attached, with pictures). Is this orthodox, done in good faith, and should I agree to it?
