I have a perforated septum most likely from nose surgery a year ago to correct a deviated septum or from using a nasal steroid to treat allergies for the last four years. The perforation is 4 mm. My ENT gave me an antibiotic ointment and said that I had a "50/50" chance of the hole healing itself. But from what I have read a perforated septum requires surgery to heal. Can it heal itself?

To lengthen your nose, cartilage grafts are used. There are a number of sources for these cartilage grafts. In order of preference, it's nasal septal cartilage, ear cartilage then rib cartilage.
