I have weighed a great deal less before and my face has never changed in being this round. I was considering buccal fat removal or Botox for jawline slimming.

Patients with "very round face" could achieve a more chiseled ( "V" shaped face or triangular face) by removing the fullness of the lower cheeks with buccal fat pad removal and enhancing their cheek bones with soft tissue fillers like voluma or cheek implants A more sculpted face usually means that the face is more "angular" and less round. Therefore areas of the face that makes it more round ( chubby cheeks) and less angular (not well defined cheek bones) should be addressed.In my practice, young patients with chubby cheeks could benefit from buccal fat pad removal if their weight is normal. They could benefit from soft tissue filler enhancement of the cheeks ( with voluma or cheek implants) to complement the result soft the buccal fat pad removal to achieve a more defined angular face.Patients interested in determining if "round face" could be improved with buccal fat pad removal  plastic surgery should make a consultation with a Board Certified Plastic Surgeon specialized in plastic surgery procedures of the face.

Dear SamH47, Better photographs would be needed to give you an expert opinion regarding buccal fat pad removal. You are correct in the fact that patients who have excessive buccal fat and would benefit from removal do not see changes related to weight loss or gain.
