Thank you for your question. The healing process does take a full year-though looking at your photo it is likely that the smaller nipple has had quicker reactivity and sensory return. It is likely that the smaller nipple can stretch to the size of the other. You may require another nipple reduction-though I recommend waiting at least another 6 months before another procedure to allow things to heal. Ensure you are wearing a bra with good support. it is hard to tell for sure without an examination. Generally speaking, areola asymmetry can be improved under local anesthesia. The periareolar incision needs to be converted into a lollypop incision to get long lasting areolar diameter reduction. The breast with the anchor scar needs more skin removed along the vertical and inframammary incision lines.

It is simpler to raise the low nipple than the other way around. Also that breast needs tightening in its lower portion. Also the lower part of that breast can be tightened easily

For the larger areola, permanent sutures will be needed to control the stretching of the areola. This area of skin stretches the most on a breast.

Unfortunately the areola can stretch even after a reduction. It is generally related to the tension on the scar. You can consider another revision to bring them in better size symmetry. Permanent sutures may help.
