Why does it seem like I have so much "mass" sagging in my low face? I have no cheek definition, bull-dog looking nasolabial folds, start of jowls, downturned lips, and a lot of volume around jowls. Have researched buccal fat removal, nasolabial fold filler, PDO threads, masseter botox, fat transfer... but I don't know what the actual issue is and what I need. I'm 31. Already time for a facelift? Why do my oblique profiles look like 2 different people?

PDO threads give temporary lift. Morpheus 8 is a good non invasive skin tightening device. You will likely need a combination of treatments to get improvements in all those areas. As we age, the effect of gravity and muscle pull stretch out the underlying skin structures (the SMAS) and also stretches the skin. Even though you are young, I believe that a facelift, necklift, and facial fat grafting would be ideal. I would recommend seeing a plastic surgeon with 6 years of plastic surgery residency training and a year of aesthetic fellowship training to ensure that you find an individual with adequate training in this very important area of the body.

Answer: Facial Asymmetry and Very Early Aging Changes You do have some facial asymmetry, but this appears within a normal variation to me. Although you do have fullness in the lower cheeks, and buccal fat removal might slim that area down, the appearance isn't that of typical jowls, and you don't show any significant skin sagging in the lower face. However, you do show some deflation in the anterior and inferior malar regions that could be improved with filler or fat injections. Because I don't see any significant skin laxity, I would be hesitant to consider a facelift. At your young age, I would stick to adding volume in the upper cheeks and removing some lower cheek fat, to create a more youthful heart shaped face.

Answer: Sagging of the lower face First of all, I would definitely not recommend buccal fat removal; it doesn’t look like you are a good candidate and it would make you look gaunt. You could be a great candidate for a mid-face lift. It is possible to undergo this surgery in your 30’ (here pictures of a patient of mine who got hers in her 30’) and some facial fat transfer. This would be the most effective method but other less invasive options could be considered (FaceTite …). An “in person” consultation is necessary to determine the best course of action based on your goals. I hope this helps, best of luck !
