Extending grafts in revision rhinoplasty

Question

I got rhinoplasty done about 9 months ago. My nose was was very flat, I had flared nostrils and my septum was deviated. During the procedure I got rib grafts placed inside my nose to shape & provide a bridge, but no trimming of the alar. Although it looks better now, I’m not completely satisfied. I expected a higher bridge and a pointy nose tip so that the ala would reduce itself as it’s pulled up by grafts. Can the columellar grafts be extended to get a more higher nose tip ?

General, the rate limiting step for achieving a very narrow nasal appearance is somebody's nasal skin. When somebody has thicker nasal skin and a darker skin complexion, the amount of definition and narrowing is limited somewhat. The best way to have your question answered with specificity would be to visit with a reputable rhinoplasty specialist for an examination and evaluation of your condition. I hope this helps. Hello and thank you for your question. Based on your photographs, you may benefit from a revision with placement of extended spreader grafts to place your tip on better tension. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.
