I got a lift and reduction 2 years ago. I want big perky boobs but I'm a 38D.Is it possible with my breast type? I want them really high the highest possible! If I can should I do it over or under the muscle? What would be best choice

Thank you for your question and photos.
Unfortunately, if you were to have implants (below or above the muscle), you will never have the result you desire for very long. The problem with the type of lift you had, which is more than 50yrs old, is that a vertical incision was made. A vertical scar where most of your breast tissue sits, will inevitably stretch and give way to the 'bottoming-out' phenomenon. Adding an implant will only make things worse for you in the long run, requiring 'maintenance' revisions to re-lift your breasts. Save yourself from further disappointment.
You may want to take a look at the breast reduction/ lift with implant blogs or questions. You will see that many women end up removing the implants altogether. It's unfortunate to see that many surgeons are still advocating implants to women with anchor/vertical lifts.

A combination of a lift and augmentation would help to improve your breast shape. The result will not be similar to the photo because the patient in the photo did not have much breast tissue. Most of her breasts are composed of the implants. Seek consultation from a Plastic Surgeon. All the best.
