I got a lift and reduction 2 years ago. I want big perky boobs but I'm a 38D.Is it possible with my breast type? I want them really high the highest possible! If I can should I do it over or under the muscle? What would be best choice

Careful pocket dissection and judicious selection of breast implant/volume will be important. Of course, careful communication will also be key. Thank you for the photos. Your breasts appear to be quite full in the bottom part, which is a very natural look for 38D. The breasts of the woman in the photo appear to be mainly composed of implants. She probably doesn't have much of her own breast tissue, unlike you, and that's why her breasts are so round on top. You could maybe get this same look if you chose large enough implants that are reasonably high profile, and have them placed in a submammary location (under the breast itself), where they will appear more round. There is a round, more cohesive implant available from Allergan, but unfortunately I don't think they are available in the US currently. Check with your plastic surgeon whether Allergan Inspira Truform 2 (Soft-Touch Gel) is available to you. Good luck!

Unfortunately, if you were to have implants (below or above the muscle), you will never have the result you desire for very long. The problem with the type of lift you had, which is more than 50yrs old, is that a vertical incision was made. A vertical scar where most of your breast tissue sits, will inevitably stretch and give way to the 'bottoming-out' phenomenon. Adding an implant will only make things worse for you in the long run, requiring 'maintenance' revisions to re-lift your breasts. Save yourself from further disappointment.
You may want to take a look at the breast reduction/ lift with implant blogs or questions. You will see that many women end up removing the implants altogether. It's unfortunate to see that many surgeons are still advocating implants to women with anchor/vertical lifts.

A combination of a lift and augmentation would help to improve your breast shape. The result will not be similar to the photo because the patient in the photo did not have much breast tissue.
