I got my 3rd set of implants 19 yrs ago to correct my natural tuberous breasts. These are full silicone and I have had immense pain for years & now a rupture. I can't bear the thought of having tuberous breasts again...I was mortified growing up and got first set of saline at age 17. Has it been long enough that my skin is stretched enough to benefit from FT w/out going back to natural tuberous shape? I was AA (if that ) before implants and these now are 460cc big C/small D.
Answer: Will explant of 19 year old implants then having lift w/fat transfer help to not have natural tuberous breasts? Hello and thank you for your question and sharing your photo. It is unlikely that you go back to your original tuberous breasts after removal. Generally speaking, after removal of breast implants, breasts will look smaller and deflated. Patients with good skin elasticity have a better chance for skin contraction and breasts returning back to what they looked like prior to their augmentation. Implant size and the amount of time since implant placement are other key factors that determine how your breasts will look like after implant removal. Generally speaking, options include removal and observation, removal and breast lift, removal and fat transfer for a natural volume enhancement, and finally removal and lift and fat transfer. You should discuss all these options with your surgeon. If you are unsure, you can just have the implants removed and decide what you would like to do after a period of observation. I would seek consultation with a board-certified plastic surgeon for a full evaluation and advice. Answer: Tuberous breasts Great question! It’s been such a long time that your breasts should not revert back to the tuberous deformity that used to have. The reason is during a tuberous breasts procedure the tissues are release and the thought is scar tissue grows in to prevent this from constricting in the future. Since it’s been several decades should not happen again. If you have enough fatty tissue then yes you could be a good candidate for a fat transfer along with the breast lift. Therefore you should seek a consultation with a few board-certified plastic surgeons and go with what you feel most comfortable with. Best of luck and hope this information helps - Dr. Roger Tsai @DrRogerTsai Board Certified Plastic Surgeon

Answer: Tuberous Breasts It is unlikely that your breasts would revert to a tuberous shape after implant removal. I encourage you to discuss your goals with your surgeon. Fat transfer does not produce the same look as implants. You want to ensure that you are selecting the correct procedure to get the results you want.

Answer: Will explant of 19 year old implants then having lift w/fat transfer help to not have natural tuberous breasts? (Photos) The smaller the implant and the less the amount of time it has been since placement, the greater the chance the breasts will return to their preoperative state. It is very difficult to predict breast size following explant b/c breast implants and weight fluctuations over time tend to thin tissues, cause a decrease in size, and create more droop. The recovery period following breast implant removal or capsulectomy because of the ruptured silicone implants will vary from one patient to another; but on average, most patients will recover within one to two weeks. It may take months for the breast to return to their former state. This will give you and your PS time to determine what type of breast lift is needed. It is unreasonable to believe that fat grafting no matter how optimally performed can achieve more than 1 cup size. 3/4 cup size increase is about average for one fat transfer procedure. My experience has been that each subsequent surgery results in smaller improvements.

Answer: Explant and lift with fat transfer The technique I recommend is explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained, if not injured during your previous surgery. Later, fat transfers can be performed if additional volume is desired. Best Wishes,Gary Horndeski, M.D.
