Droopy eyelids. Would fillers or fat injections help? (Photo)

Question

I’m 51 and have terrible dropping of my lateral eyelids. I’m afraid to do anything that removes skin because my eyes don’t close all the way and I have very dry eyes. Wondering if fillers or fat injections could help? I also read that sometimes the muscle can be tightened without an incision? I get Botox and have done Sculptra and some filler in temples, but the crease in my lateral eyelid makes me look so old.

Hello @Imaginative626046, thank you for your question. The lateral eyelids do not exist; they are inferior and superior. A thorough evaluation is necessary if the eye does not close completely, since there is a lot of skin on the upper eyelid. Eyelid lifts can be performed by simply removing the excess skin. As we age, our eyelids naturally sag and droop. A combination approach would be best to see visible improvement. In terms of non-surgical treatment options, I would recommend a Chemical Brow Lift (using Botox, Dysport, Xeomin, or Jeuveau) and filler injections to rejuvenate the temples, eyebrow, eyelid, eye troughs, upper eye hollows, and forehead. I typically combine this with Ultherapy and thread lifting to lift the brow; RF and skin tightening laser treatments to stimulate collagen production and rejuvenate the skin; and laser or superficial RF devices for improvements in texture, color, and any crepey skin around the eyes as needed. You seem to be a great candidate for an upper blepharoplasty. To clarify, an upper blepharoplasty is the removal of excess skin from your upper eyelids. Since it is only the excess skin that is being removed, it should not alter the movement of your upper eyelids. Nonetheless, I would recommend you to complete multiple in office consultations with board certified facial plastic surgeons to discuss your ideal outcome. Neither filler or fat will help your upper eyelid droop (ptosis). You do, however, have a significant lack of fat and slightly medial to central. This could be best treated with fat injection. Your other problem is simply ptosis or droop. This requires surgery for permanent correction, though you could treat it with a eye drop. Particularly because of the problem closing your eyelids, make sure you see a plastic or ophthalmic plastic surgeon who is significantly experienced in ptosis treatment.
