Thank you for your question. It is hard to provided with a valid medical opinion without photos.  I would suggest to be treated by board certificated plastic surgeon with experience in this procedure, to get the best desired results.

This is really more about the source of the nerve pain, particularly if this is result of herniated disc or something remote from the location of BBL.  The sciatic nerve is easily avoided in fat placement,  as the injections are performed superficial to it.   You would need to obtain clearance from the physician who is treating you for the pinched nerve to have the BBL.
