There is no strong evidence, suggesting that PRP improves fat transfer results. I suggest focusing your efforts exclusively on the two single variables that matter when it comes to getting consistent quality results for this procedure. The first is understanding your own candidacy for the procedure, including what the procedure can accomplish, and what it cannot. The second variable is choosing the best provider. Choosing the right provider is far more important than adding plasma, which is unproven to have any benefit. Delivering consistent quality, Liposuction and fat transfer results is more difficult than most people believe it. The number of plastic surgeons with mastered both Liposuction and fat transfer is in a relatively small minority. The results of this procedure are permanent and irreversible. Ignoring the importance of choosing the right provider by having multiple in person, consultations and properly vetting plastic surgeons puts patients at risk for having permanent deformity. Because of the complexity of this procedure, and the number of providers lacking talent, I suggest patients start by having a minimum of five in person, consultations before choosing a provider. For some patients, they may need more than five consultations to find a sufficiently talented and experienced provider. Breast fat transfer is reasonably good at increasing breast size by approximately 1/2 breast cup when done correctly by an experienced surgeon. The procedure does not change breast shape in any significant weight, and patient should have a clear understanding of it, if the desired result is simply increasing the size of the breast, or if need a change in shape. Unlike implant augmentation which drastically changes the shape of the breast fat transfer does not do this. To get a clear understanding of what the procedure can, and cannot accomplish it is imperative that patients understand how to view before and after pictures. Early postoperative pictures of fat transfer results can look very impressive but early results do not represent final results. The majority of volume seen early after fat transfer is lost over the first several months. Results do not represent final results until a minimum of 3 to 6 months after surgery. Do not assume providers are showing you long-term follow up results when showing before and after pictures. During each consultation, I suggest you ask providers to open up their portfolio and show you their entire collection of before, and after pictures of previous patients, who had similar breast and body characteristics to your own. Confirm specifically the timeframe of when after pictures were taken for each case. Get familiar with what quality results should and should not look like based on knowing when after pictures were taken. Learn how to judge not only fat transfer results, but liposuction results. The ideal candidate for this procedure should be a good candidate for both Liposuction and fat transfer. Patients who are looking for dramatic changes, or a change in breast shape like getting upper pole Fullness in the breast should consider implant augmentation, which is a superior operation for the majority of patients seeking breast enhancement. Patients should understand their own candidacy for both Liposuction and fat transfer since this is a combined procedure, doing two operations at the same time. Generally speaking the ideal candidate, for the procedure tends to be young women with tight skin, who have slightly excessive amounts of subcutaneous fat, especially in the torso with appropriate breast shape that would be enhanced by the addition of modest volume enhancement. An experience plastic surgeon should have no difficulty showing you the before and after pictures of at least 50 previous patients. Being shown a handful of pre-selected images, representing the best results of a providers career or being shown examples of early postoperative results is completely insufficient to get a clear understanding of what average results look like in the hands of each provider or what final results may look like. Transfer often comes from patients who did not focus enough effort and properly vetting plastic surgeons in regards to Liposuction skills. Skilled and experienced providers know that results are based purely on patient, candidacy, and the skill of the surgeon. Trying to convince patients to choose a provider based on having some technical advantage based on equipment and technique is a fallacy I suggest patients avoid. There’s no correct number of consultations needed to find the best provider. In many ways, some of it is simply due to luck, or being intelligent about the pre-selection process. The more consult patients have a higher chance of finding the right provider. Considering the number of patients who are dissatisfied with the outcome to the point of being left permanently disfigured, I cannot emphasize enough the importance of patients being more selective than they think necessary when it comes to choosing a provider. When, in doubt, slow down and schedule more consultations. The majority of patients seeking breast enhancement should not exclude implant augmentation as being one of the best options.
