You do not need a breast implant nor the use of a foreign material such as ADM to get a good result from mastopexy.

Hello,Unfortunately, there is no evidence to suggest that the outcomes of breast lifts with biologic materials changes the natural course of descent that occurs after a pure lift of a large breast.  Quality and durability of a breast lift is directly proportional to its final size, with smaller and lighter breasts having longer lasting results. Best of luck!

Thank you for your question. This is a very forward thought process! The answer is yes, there are options out there for an internal bra using ADMs or other types of soft tissue support devices. You can be a good candidate for this is you have stretched soft tissue that would be at risk of re-stretching over time. You should discuss your desires with your plastic surgeon and get his or her opinion regarding this approach. Also may be best to get multiple opinions/consults for this type of thing. But be aware that these materials will come at an additional cost and can be quite expensive in a cosmetic setting. Best of luck!

You may want to consider the Breast lift with Auto Implant technique. This is also referred to as a Mastopexy with Auto-Augmentation. However, instead of using ADM you may do well with galaflex mesh. Keep in mind that while the addition of the internal bra or mesh is not necessary, it can help enhance the results.
This is a powerful way to improve breast shape, and give the appearance of increased breast volume without the use of implants or multiple rounds of fat injections. 

The breast lift with auto-implant uses a lolipop incision and repositions sagging breast tissue (below the nipple) and rearranges this tissue to increase the volume of breast tissue that is behind the nipple. Traditional breast lift techniques discard this "lower pole" tissue but I prefer to utilize this tissue to artistically and increase volume to give a healthier, perkier and natural appearing breast. It is an excellent way to provide a modest increase in size at the same time as a breast lift.

can be done but if its advertised as better and longer lasting, see if the surgeon will put that in writing with a guarantee that allows for free revisions for so many years.  I doubt you will find that.  I am not a fan of ADM's because they come with side effects, some that mimic infections.  I prefer to use your own tissue like in a Rubin mastopexy or autoaugmentation mastopexy.  As for nipple sensation, no one can guarantee that you may not lose it with surgery.

The auto-implant technique as described by Dr Graf is a method of redistributing lower pole breast tissue to the upper chest wall using a strip of the patient's own pectorals muscle as a sling.  Many of us have used a strip of Strattice to make this sling in the same fashion without disrupting the pectoralis.  It is not a bra so much as a belt. 

Yes, we commonly use ADM in our breast reduction/lifts if we are working with heavy tissue. You'll just need to see plastic surgeon who are comfortable with this procedure.
