I am 56 with large breasts that are drooping significantly. I do not have implants. I would like to achieve 1) a lift, 2) a reduction, 3) maintain nipple sensitivity, and 4) utilize some kind of "internal bra" (using ADM?) to achieve a longer lasting lift. It seems most surgeons using ADM in breast lifts do this only in conjunction with implants. Is there a PS out there doing a reduction/lift along with some form of "internal bra" WITHOUT also using implants? Your thoughts much appreciated.

The answer is yes, there are options out there for an internal bra using ADMs or other types of soft tissue support devices. You can be a good candidate for this is you have stretched soft tissue that would be at risk of re-stretching over time. You should discuss your desires with your plastic surgeon and get his or her opinion regarding this approach. Also may be best to get multiple opinions/consults for this type of thing. But be aware that these materials will come at an additional cost and can be quite expensive in a cosmetic setting. Best of luck!
