Your breasts are too long on the chest wall for augmentation alone. The technique I recommend is a lift using The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Later, fat transfers or small implants can be placed if additional volume is desired.

In person exam is essential. Based on limited photos, it appears that your breasts sag and your nipples point down and it will require a lift with possible implants ( if you want them fuller) to achieve the best result. I suggest that you seek an experienced Board Certified PS

I almost always place implants submuscular. It lowers the rate of capsular contracture significantly. In addition, it looks much more natural because the muscle provides covering over the implant so its not as round on the top. I've also noticed the implants drop less over time when they are protected under the muscle.

An in person evaluation is best, but from the submitted photos it appears you will have the best result from a lift with implants. This would give you the fullness you are wanting, improve asymmetry, and raise the nipple to the correct position. Implants placed over your muscle will be a better option. Because you already have a breast ptosis and you do not want a lift. So if the implantsa are placed under muscle, then you will probably have a “Waterfall Deformity” which means your breast will look like pouring down over the implant. So subglandular, or over the muscle is the better option for your situation.

Yes, you can place implants over the muscle and no lift, but you will have larger and still sagging and uneven breasts. a lift is needed to correct the sagging and make them even. If you choose a lift, I recommend placing the implants behind the muscle for better support and upper breast fullness. I would suggest doing the lift so you will not be disappointed that no lift was done.
