Based on your singular photo, you may have a piece of bone that is causing your philtrum to look funny. This appears to be associated with a long nasal appearance, a hanging columella, excessive nostril show, and an underprojected nasal tip appearance. The philtrum actually appears to be well preserved. The lips and nose are harmonious. Any surgery on the philtrum can lead to significant scarring. It is important to avoid it unless there is a gross abnormality of the philtrum such as cleft lip, or cancer.
