Are nipples placement to high ? I had ask my Dr and he says tattooing will be the best approach and to be honest my nipples has been peaking out my breast with every movement I do, making it very embarrassing for myself.

Your concerns are understandable. Your situation can be improved upon with removal of additional skin/tissue along the inferior and lateral breast fold areas. This revisionary operation will help to better center the nipple/areola complexes on each breast mound and hopefully reduce some of the "peekaboo" effect of the nipple/areola complexes when you are wearing bras, swimming suits… Given your plastic surgeon's response, you may be best off seeking second opinion consultations with other well experienced board certified plastic surgeons.
