the surgeon had put in dissolving stitches the first time. He re-did the procedure about 8 months later and used permanent stitches. He did not remove any cartilage either time. The ears are slipping back towards original appearance, at the tops, in the last couple months. The left ear is back out where it was at the top before surgery. Right ear has slipped out about 1/16 inch. left ear cartilage is much stiffer than right ear. What should i do? this was and is important to me

You ear is pinched at the top. But what is most visible is the point. The easiest thing would be to excise the superior aspect of the pointed ear and connect the two resulting helixes. This would result in better contour.
Other surgeons would be correct to critique...

At one month, any swelling should have resolved. Your history does sound suspicious for a perichondritis, infection of the cartilage of the ear. This can be serious and disfiguring without proper treatment. You should seek a consultation with an Otolaryngologist to determine how to proceed. I...
