Are these perioral mounds or something else (pre-jowls?)? I feel the pockets around my mouth area contribute towards aging my face compared to the rest of my features. I am 37, although I have always had some degree of these mounds since my early 20's...age/gravity is making them more pronounced and while I think I am still too "young" for a facelift, I am curious if careful lipo or even fat transfer could help. I'd like to avoid traditional filler or RF treatment.

Liposuction or fat transfer will not help. There is not a measurable fat pocket present. It is too far anterior and inferior to be buccal fat which is visible in the mid cheek compartment of a deep plane dissection. It is more likely due to a muscle prominence called the modiolus, which does not have a good solution except possibly with lifting.

That may be buccal fat.  Excision can be done in the clinic under local anesthesia.  The incision is made in the mouth and heals very well typically.
