Question

I am now in the consultation phase of my journey to correct jowling , nasolabial lines, and transition between under eye and upper cheek. I am open to a mini lift, a cheek lift via lower blepharoplasty incision, or facial fat transfer and any other appropriate and safe alternative besides fillers. Which would be the procedure that could address my concerns with the least amount of risk for complications and scarring? I am 49 and have already had a lateral brow lift.

Thank you for your question and for provisioning photos. Mini facelifts are ideal for treating mild to moderate skin laxity and jowl formation. During this procedure, the jawline can be angulated, jowls reduced, and excess skin removed, while the remaining skin is tightened to create a refreshed and rejuvenated appearance. 
When compared to a traditional facelift, mini facelifts are less invasive and can require a shorter recovery phase while providing results that may last for eight years or more in some patients. I recommend scheduling an in-person consultation with an experienced, board-certified facial plastic surgeon. They will be able to conduct a physical exam to determine the best procedure for you.

Keep this simple. A basic facelift done by a well respected surgeon who gets good results. I’m not a fan of fat injections. They do nothing but make your face swollen until the fat resorbs and waste money.

Thanks for the posted photos, it really helps. The issues are you can not have it both ways... Surgery like mini FL/NL SMAS plication with either nano fat grafting in combination can leave scars! Best to do virtual consults with....

Your photos don't show anything resembling a tear trough in the lower lid/cheek junction, no nasolabial folds either and no jowls. So I would recommend doing nothing to these areas as it will be all risk and no benefit. But your neck skin does show some laxity and potential for improvement with a lower facelift or neck lift. But there are scars associated with those procedures so you need to weigh that as well. Maybe just keep up with medical grade skin care for a while.
