Your skin laxity will not be addressed with a platysmaplasty alone. It must also be accompanied by lateral pulls from a neck ir facelift. This approach obligated the need for incisions behind the ear.
Getting the loose skin under the chin tightened up is best addressed directly by a platysmaplasty under the chin incision.
Hello and thank you for your question. Based on your photographs and neck laxity, you may benefit from a mini facelift with a neck lift. If good technique is used, this can be done with minimal scarring and can result in a natural look, not over-done look, with rejuvenation of your face and neck. You will likely need more than incisions only behind your ears. Usually this involves a combination of platysmas plication and skin excision sometimes in the submittal area but also laterally around and posterior to the earlobe.
The photographs demonstrate significant laxity in the neck which will require lower face and neck lift procedure in order to improve that area. A lower face and neck lift accomplishes tightening loose facial and neck skin, tightening loose facial and neck muscles, lifting the jowls, and removing any fatty deposits in the neck which also includes a platysma plasty. The incision is very inconspicuously placed in front and behind the ears.
