I had a consultation with reconstructive oculoplastic surgeon. He exterminated my lower eyelids very carefully. He proposed me the next option. According to clinical presentation of my case-he said that i need to reinforce back lamella by Hard Palate Graft 2 steps(from conjuctiva site). But first of all he said to operate right lower eyelid. After couple month of rehabilitation-left lower eyelid. Dear doctor-why? Do you have any suggestions? May he wanted to achieve the best symmetry of both my eyelids?

You have cicatricial lower eyelid retraction. It requires Soof/midface lift and possibly internal spacer graft which could be hard palate, alloderm or other. You have scleral show after your blepharoplasty.  I believe that this could be corrected in a variety of ways but the simplest would be the use of a tarsal strip procedure.  This could be done under local anesthesia , with little down time and you won'd have to take tissue from another area.

You will hear different opinions on correction of the lid retraction and scleral show after lower blepharoplasty. It seems you had a transconjunctival bleph WITH skin removal?? The after photograph is in up-gaze which tends to show more sclera below. If this is bothering you, I would seek a second opinion (i.e., other than hard palate grafting). I believe, just based on your photo, you have sufficient tissue for a normal lid level without adding a graft, and that a more straightforward procedure to release the septal scarring and perhaps tighten/ support the lateral canthus would be effective. Both eyelids could be repaired at the same time. Best wishes.
