I would recommend placing the implants above the muscle .and fixating the inframammary fold with mesh

Please note that this method can be applied to some breasts, but not all. In your case, some breast tissue in the shape of an apple slice should be removed from the lower part of the breast. Consequently, the breast size will be reduced. While the nipples can be repositioned, this is a challenging procedure.
