Puffy nipple reduction? Breast augmentation 18 months ago made them larger

I've always had puffy nipples. I had a breast augmentation 18 months ago which made them much larger, lighter, more puffy and very prominent, especially if I’m not wearing a bra. I was not expecting that. Now when I bend over the puffiness makes my boob look triangle shaped. Is there a way to make the nipple flat? I want them to look they way they look when they're hard and tight, but I don't want to lose sensation.
