Hayley S  Thank you for the question.  Unfortunately your photo alone is not adequate to make a comprehensive plan.  An in office consultation will be best to develop your treatment plan.  First of I believe your best course of action is filler.  Your cheeks need to be built up structurally  and then proper placement of filler can be utilized in the tear trough area.  Fillers like Voluma and Bellafill would be perfect to  build the cheek structure.  The tear trough could utilize Belotero or Bellafill ( for longer lasting).  You will find that each cosmetic surgeon may utilize different fillers to achieve the same task.  As far as surgery there are implants that can be used but with greater risk then with filler.  through eyelid surgery alone ( in studying your one picture) it appears it could be difficult to achieve  aesthetically pleasing results.

You have the standard tear trough and I am surprised your doctor said that. The vast majority of these cases in North America are currently treated with a filler whether it be fat, hyaluronate or something else that is injected through a needle or transposed into the trough during surgery.

There are various options for sunken hollow under eyes with dark circles. 1) Filler injection (see video below); 2) fat injection; 3) tear trough implant. See an oculoplastic specialist.

The best and lasting option for correction of hollowed eyes is fat injection. Not only fat brings volume to the eyelids, since it contains stem cells, it provides the eyelids a natural youthful glow.

I have helped may patients like you with hollowness under their eyes either due to genetic (your case) or over corrected lower eyelid blepharoplasty. Resylane or Voluma can help you very well.  

"Bags" or "hollows" or even "dark circles" are all common terms people use when they have recessed tear troughs. In most cases hyaluronic acid fillers such as Juvederm, Restylane, Belotero or Emervel are great options to fill the volume loss in that area. In some cases, patients may experience bruising since the skin in that area is very thin, but results are great. Blepharoplasty surgery may also be an option if you are a candidate.Blepharoplasty is eyelid surgery that is done to remove excess fat, skin, and muscle that has made a home around the upper and/or lower lids. This procedure does not get rid of wrinkles that lie around the sides of the eyes, but it can improve dark circles and the overall appearance of the area. Consult with a board certified plastic surgeon to determine your best options. A second opinion wouldn't hurt.

A recessed infraorbital rim creates the deficiency along the bone that you see under our eyes. Injectable fillers or fat injections would be the standard treatments offered. But a lower eyelid approach to place either tear trough (medial infraorbital rim implants) or complete infraorbital rim implants offer a permanent solution to structural under eye hollows.

You need volume enhancement of your tear trough.  I would suggest tear trough correction w/ Restylane as a first line treatment.  It is easy, has minimal downtime and is reversable.
