Thank you for sharing your question and photograph. It appears to be a multifactorial issue of both strong depressor lip musculature and volume loss. As a result I would recommend small amount of Botox and dermal filler placement for optimal results.

A downturned mouth can be improved with both filler and botox. The botox will relax the muscles at the corner of the mouth which naturally tether it down. The filler can add some support to the tissues in the corner to prevent further drooping. Hope this helps!

You can have botox injection to the muscle that pulls the sides of your mouth and or add a hyaluronic acid filler. Combination treatments work well.
