Hello and thank you for the question. This is a medical decision. The choice between local and general anesthesia, is best made by your doctor who understands the pros and cons of each approach. Having said that, there is a lot you can do under local. A BBL is not one of them. Liposuction is painful and difficult to achieve with just local anesthesia alone. The problem is made worse, when having to administer higher and higher amounts of lidocaine to help you not feel pain. At some point, lidocaine can be dangerous and toxic. In my practice, I see many patients who have had liposuction under local and are not happy with the outcome. They complain of the pain during the procedure, and the results being uneven. The upper abdomen / back area are notoriously difficult to get good anesthesia and due to the more fibrous fat, more difficult to remove. Patients can and should be fully involved in the decision making process, but they should trust in the experience of their doctor to make the best decisions on the technique and methodology of the procedure. Yes, you will feel some lipo and fat injection when only local is used. I prefer in most cases for a Bbl to use general.
It can be done under local if you inject only above the gluteus maximus muscle, but to get the best result, you need to put fat above and below the muscle. Most practitioners who do BBLs under local anesthesia are non-plastic surgeons without hospital privileges for these procedures.

I would say that no we do not like to do fat transfer under local anesthesia. because we are injecting the fat deep into your tissue it is much to painful and we do not use local anesthesia for this procedure.
