What is this crease in my mid cheek? I'm not sure if it's malar bags or pockets of fat,

It is a fairly common occurrence, usually in older individuals, but sometimes even seen in very young persons. Happily, it only takes a matter of minutes for a small amount of filler to be instilled directly under the crease to elevate it and to make the cheek one continuous convex surface.

Answer: Line on face You are seeing the line between the lower eyelid and the cheek. It runs down just under the malar fat pad. It is more obvious in people who have a large or edematous malar fat pad. You can fill in the line with an HA filler if this bothers you.

But it does become more noticeable as you age. It can be helped with filler or fat transfer but does not require surgery
