Ive lost a lot of volume under my eyes since taking accutane 4 years ago. I’m considering getting a rhinoplasty, however my concerns are that it might sink in myunder eyes even more. I am also concerned about the fat pads along side my smile Lines, its dragging down my face. Is there a procedure I can have done to correct this along with a rhinoplasty? Thanks!

Answer: Work on the nose would not change those other two areas. More ... Although, sometimes, changing what the nose looks like can change how you *perceive* the under-eye area or the smile lines, but they almost certainly wouldn't *actually* change. Especially since, as I see your nose from these images, it would be mostly the tip that is altered, right? You didn't post any three-quarter views, and it's those 3/4 views that usually best show the width of the tip. Finally, remember that rhinoplasty is an exquisitely difficult operation to get right, and you should only have surgery if you are able to make yourself very confident in your surgeon's skills. The changes that your nose needs, particularly work on the tip, require advanced techniques, and skill that most plastic surgeons don't possess with expertise. For most noses, it's much better to not have surgery than to have inexpertly-performed surgery. Read my essay on how to stay out of trouble while selecting a rhinoplasty surgeon. And it also discusses how to take photos that are best for online evaluations.

Answer: Rhinoplasty surgery should not change the appearance of your under eyes. That said, you appear to have relatively thin nasal skin and an overall pleasing nasal appearance. Since you didn't mention what's bothering you about your nose, it's not clear whether rhinoplasty surgery is a good idea. Insofar as your under eyes go, injectable fillers may be helpful. I hope this helps! Sincerely, Dr Joseph
