Restore dorsal hump, restore wider nostrils? Please help, I miss my old nose.

You can add gears which can restore a dorsal hump, lengthen a nose or widen a nose but unfortunately there is no way to widen nostrils without a skin graft which would leave highly visible scarring.  I suggest consulting with a surgeon experienced in revision rhinoplasty to find out what is realistic.

Rebuilding a dorsum that was over resected or botched and re-expanding nostrils which look pinched due to also structural issues is feasible with structural revision rhinoplasty techniques.Seek the advice and hands of a serious professional who offers to you a time-consuming and expensive procedure; stay far from minimally invassive snake-charming procedures, synthetic implants, fillers, wrapped diced cartilage, etc.

If you wish better grounded opinion well lit, focused and standard images have to be assessed: frontal, both lateral and both oblique views, also from underneath.

I recommend getting a consultation to go over your concerns and your desired results. Revision rhinoplasty is a technically challenging procedure and I would suggest that you seek a surgeon that has a good track record for natural results. There a few select of these surgeons in each major city.  I would look at their reputations, board certifications, and before and after pictures.

There is an old saying -- you cant go home again. Once tissue has been removed, you cant add it. Perhaps you  can have some grafting to the dorsum to get a bump -- but after your alar bases are trimmed, that is it.
