I believe that the only thing that will significantly improve cleavage is an implant, unless you have very large breast and a reduction/lift is performed.  I would recommend an
in-office examination as well as a detailed discussion with a board certified
Plastic Surgeon.

Depending upon your anatomy, it may or may not be possible to enhance your cleavage. This usually requires implants with a breast lift. In some cases, extra cleavage can be enhanced with fat grafting.A lift alone will not restore upper fullness. It would be helpful if you can attach pictures to your question. A breast lift is a great option for you since you are not looking to enlarge your breasts, but a breast lift can not give you a full round upper pole of the breast, and if that's the look that you're going for then you may need implants. A breast lift will enhance cleavage to a certain degree , but it is possible that if you have a very wide breast bone then you may require additional fat grafting for a more pronounced cleavage. From your description you need a lift.  What it will accomplish depends on your anatomy, skin quality and breast volume.  Usually to get upper pole fullness an implant is still needed with a lift.  Since you are only 20 years old   and already have saggy breasts, you have loose tissue and skin which will make you more likely to need another lift in the future.  I would suggest you look into having some alloplastic material such as acellular dermis(Strattice) placed at the time of your lift to help make it last longer.  Again cleavage may depend on your anatomy more that the surgeon's technique.

A lift would address part of your complaint, namely the sagging. Congratulations on the weight loss! However, the spread between your breasts is anatomical and this won't bring them closer together.  If your breasts are already full, then an implant would not seem to be an essential component of a proposed surgery. Unfortunately, the cleavage is related to your overall anatomy. One can not change that too much with a lift or with implants.
