I'm a 24 years old now. I've had sagging breasts since I was 12 years old. What should I do?

my right sided breast is larger than left one and it's more saggier. I always feel it touches my body when I'm not wearing a bra.This is really uncomfortable to me and I always wear a bra all over the day even when I'm sleeping. Recently I noticed there is some kind of rash on both of the nipple areas of my breasts. And I'm taking medicines for diabetics since 2011. What should I do? Can I do a surgery to correct them? the main problem is my blood sugar level is not controlled well.
