I'm 42 and my weight has fluctuated several times over the years. Biggest weight loss was about 70 pounds. 5'4 and about 168 now, do I look like I'm ready to have skin removal surgery now based on my stomach area? I'm not sure what weight I should be because I haven't been an ideal weight for years.
Answer: Tummy tuck and breast lift Thank you for your question and congratulations on your 70lb weight loss. I always recommend that patients be within about 20lbs of their weight goal. With that being said even if your not in that range you can still have surgery just understanding if you have a substantial weight loss after surgery your surgeon may have to tweek something later. Looking at your photos I would suggest a tummy tuck and breast lift. A tummy tuck will allow your surgeon to tighten up your abdominal wall muscles from the weight gain and loss and remove the extra skin. It will give you the flattest stomach that one can give. You may have some intra abdominal wall fat, which is fat around the intestines. That fat is the fat you will lose with weight loss. Most surgeons will also do some liposuction on the flank area, also understanding that you may need some additional after surgery because they can't be too aggressive at the same time. Regarding the breast I believe you would be a great candidate for a breast lift. This will allow your surgeon to boost the breast up and if you choose to add an implant under the muscle to help with the shape you may. It would be useful to see full pictures of your abdomen with no clothing. Based on these photo, I believe you are a good candidate for a standard tummy tuck. Extended tummy tuck is really not a different surgery, rather just a tummy tuck with slightly longer incision that is needed to give you maximum skin tightening. During tummy tuck, extra skin in the lower half of your abdomen including the stretch marks will be removed. In addition, typically the fascial laxity/separation of recti muscles will be taken care of by muscle plication, giving you a flat abdomen. The added liposuction of your hips/flanks will help with creating a natural and aesthetic waistline. Incision can be placed very low to be hidden under bikini. With proper surgical technique, belly button can be designed to be a true 'innie'. Recovery from tummy tuck is 1-2 weeks. Strenuous activity and lifting heavy weights should be avoided 6 weeks to 3 months post op. I would seek consultation with a board-certified plastic surgeon for a full evaluation and advice.
