I want to reposition my under eye fat, to help fill in the sunken appearance of the tear trough. I only have puffiness when I smile, but not when my face is at rest. Is this surgery good for people without eye bags, and just needing to fill tear troughs? (I don't want filler)

In order to achieve more positive canthal tilt, you may need lower eyelid retraction surgery with canthoplasty. Best to have personal evaluation to determine. See an expert.
Thanks for your question! It appears as though the brow is lower on that side which appears to be making the lid look droopier. I would suggest seeking opinions for a unilateral brow or forehead lift. Good luck!
