My (upper) face has widened dramatically over the past few years - could this be muscle, fat or bone growth/thickening? How can I achieve a slimmer face?

There are actually a number of ways to achieve a slimmer facial profile. And, like all things plastic surgery, the right one is the right one for the goals you're trying to achieve. Things that can be considered are buccal fat pad excision for slimmer cheeks and the lower third of the face, bony reduction of the jaw or chin, called genioplasty, or if teeth alignment is an issue you can even explore orthographic surgeries, which realign that upper and low jaws relative to each other. Other modification of of the face that are more well-known include a face and neck lift, neck liposuction, or Kybella to the neck. There is a combination of options or single option that is right for you. Your upper face appears wide because your lower face is narrow.  It is very difficult to reduce the width of your upper face but you can consider doing a procedure that will make your lower face more harmonic.  You should consult with a real expert in facial plastic surgeries.
