Will a columella plasty make my nose look retracted or sharp? Will it become slightly rotated? (Photos)

Before continuing please note I do not want and i am not interested In a full nose job. I am scheduled for a columella reduction but i am a little nervous. I dont want my nose to be sharp looking or retracted because the tip of my nose is sort of round (which i dont mind) As far as a little rotation i wouldnt mind that as i read sometimes with a columella reduction it may happen. But I am not sure if my tip cartilage is the problem or if it is columella.i am looking for the smallest change possible that others may not even notice But i will know changed. Some pictures it looks like it’s the columella sometimes it doesn’t.

Based on your description and photos, your concern appears to be a mild columellar show rather than a significant issue with the nasal tip cartilage itself. It’s important to understand that very subtle changes, such as a conservative columella reduction, can refine the base view without creating a sharp, over-rotated, or “operated” look when performed carefully. Your goal of achieving a minimal, natural improvement is reasonable, and this can often be done without a full rhinoplasty by using a conservative approach that preserves your existing tip shape and avoids excessive rotation. The key is precise surgical planning to maintain balance and softness of the nose. A detailed consultation with an experienced surgeon will help confirm whether the columella alone is the main contributor and ensure that any adjustment aligns with your goal of a subtle, almost unnoticeable enhancement.

On your photos, you appear to have a wide nasal tip appearance, with relatively thick nasal skin. On profile, I agree that you have a hanging columella, however, this is associated with a droopy nasal tip appearance. You may also have a small dorsal hump. Hello and thank you for your question and pictures. You actually have a very difficult problem to correct- a long nose with a hanging columella and excess nostril show. Unfortunately, a minimal procedure or surgery "just to the tip" or "just to the columella" will not be able to fix your issues- any surgeon that tells you otherwise is either inexperienced or is intentionally doing you a huge disservice. A rhinoplasty can best address your aesthetic concerns and any functional issues with your nose. I would be able to adjust the size, shape, and especially the definition of your nose (particularly the tip and columella), improving your overall facial balance. I'd also be able to open up and solidly reinforce the underlying structural framework of your nose (to prevent distortion and retraction from scarring forces over time)! This would truly be the best option for you.

Rhinoplasty truly is a surgery of millimeters- and whether you're looking for a minor adjustment (as in your case) or a major transformation, it requires exceptional surgical skill to achieve functional and natural-looking results! Because your nose is the DEFINING feature of your face- I would only recommend seeing a surgeon who specializes in rhinoplasty procedures, and who has significant training and experience performing these surgeries. I actually trained under one of the world leading rhinoplasty experts (who performs 1,000+ nosejobs annually).

Thank you for your question.  I believe the columella reduction by itself will just leave you with a retracted columella.  I have been in practice for over 30 years and rhinoplasty is a large portion of my practice.  You are an excellent candidate for rhinoplasty.  I think any shortcut that you try to do will end up making your nose look worse rather than better. Best wishes to you.
