What would need ti be done to give me less lateral mass & more properly placed nipples & less bottom heavinesd. I have 545 cc currently.. could positive changes be done with out replacing the implant

breast revision surgery is the most difficult plastic surgery procedure performed. Botched breast surgery alters normal tissue planes and laxity, the ability of breasts to heal properly, and leaves scar tissue that dramatically affects the level of difficulty of the corrective surgery.
Many patients come in to correct poor outcomes from their initial surgeries. The most common problems are due to implant malposition, bottoming out of the implant, poor scarring, and incorrect implant size.
While the type of revision required will depend on the result of the previous surgery and the desired result of the patient, some revisions involve extensive pocket repair that may or may not need specialized external bras.
If you are considering breast surgery revision, you should speak with a board certified plastic surgeon and have yourself properly assessed. Only after a thorough examination you will get more information and recommendations. Also, you have to make sure that your surgeon understands your breast augmentation goals.

Seems quite clear the implants you wear have horizontal dimensions far exceeding the limits of your chest, hence deforming the breast lift patter by finding their way out of it.

Solution: full revision of the mastopexy pattern and better choosing of implants dimensions (which does not equal to have more or less breat volume).

Now your nipples are, indeed, eccentric to the breast mound, also because of the issues sourced on the breast lift pattern and the dimensional conflict of the implants.

Based on your two posted photos, best to do a virtual consultation, but revision of the breast lift specifically on the lateral inferior areas with possible mesh added for strengthening and slight relocation nipple complex is a possible fee is $15,000-$25,000. Best of virtual.
