Boxy shaped breast after breast reduction surgery, which procedure would help? (Photos)

The insurance company does not pay the surgeon to address the large lateral chest wall fat rolls, thus, he did what your insurance covered: give you smaller breasts. What procedure would help to give more aesthetic breasts? You do have asymmetrical breasts with large fat rolls on the lateral chest wall, and you will need additional reduction to make them symmetrical and liposuction to contour the lateral breasts and chest wall. This is not considered a "touch up", it is a new operation to correct your results. This is not considered a good result nowadays. Although your result would be considered a win 25 years ago, the level of aesthetic refinement has progressed. Your scars are great, but your symmetry is terrible. Find a surgeon to perform a revision surgery to address these issues so that you can have pretty and symmetrical breasts.

You might want to consider liposuction of the outer portion of the breast and the fullness under the armpit. This would be worthwhile

If you’re dealing with a boxy shape after your reduction, a revision surgery might be necessary. In general, the wise pattern (inverted T, anchor incision) is more likely to cause this. Sometimes the breast can be reshaped while other times it is more challenging. Seeing a plastic surgeon to evaluate and discuss your options and timing may be helpful. If your original surgery was many years ago, there are more options available for revision surgery.

You could have a revision to remove some excess lower and lateral breast tissue. Insurance may not cover this since the amount removed may not be enough to qualify. Please discuss this with your surgeon.

You may want to consider a revision to your breast reduction and include autoaugmentation. This is an approach that uses your own tissue to give upper pole fullness to the breasts. This is different than fat grafting. I've included a video that I hope you find helpful.

To make an assessment regarding the outcome of any plastic surgery procedure, we generally need to see a complete set of proper before and after pictures.
If we don’t know what you look like before, we can’t really assess the quality of the outcome.
If you don’t have before, and after pictures, then ask your surgeon to forward the pictures they took.
it looks like you need to have the envelope of your breast tightened further.
Your surgeon may may have limited how much tension they put on the incision lines during the first procedure in order to minimize complication rates which are fairly high for breast reduction surgery, especially for women who have very large breast or breast that hang very low.
