Over the past year I’ve lost 60lbs and turned 30! With all of this in mind I noticed significant volume loss throughout my face. I recently saw a plastic surgeon who agreed and recommended fillers for my temporal area, jowls, under eyes, and nasolabial folds. I received Botox and 6 syringes of fillers. I definitely see a change, but I still notice sagging in the nasolabial fold and jowl area. I’m not interested in surgery. Would additional filler further smooth out the area/less sagging?

In terms of adding more volume with fillers, you bring up a good question. Adding a little bit more filler at your nasolabial fold may help augment your mid-face. Be careful not to over-do it, as it can sometimes be unnatural or migrate. The nasolabial fold in general can be risky if not done appropriately since there is a blood vessel nearby. In terms of the jowls, adding filler may be able to blend the jowls, but again if it is over-done it will look unnatural and may be counterproductive. Putting filler is adding volume, so anything that increases volume in the lower face makes the face look heavier and older/aged - so be careful. Biostimulators like sculptra or radiesse may be an alternative to fillers for the mid face and temple. Talk to your plastic surgeon to see which option is best for you. I hope this helps!

Botox or any neurotoxin can help tighten jowling/jawline if placed in the DAO and playtsmal bands. These are both depressor muscles, which means they pull downward. If neurotoxin is placed here, it will help “elevate” the muscle instead. If interested in additional filler, you could potentially treat chin shadowing, add chin filler, as well as cheek filler which can help pull back nasolabial folds. For a thorough consult, I recommend seeing an experienced practitioner who could ultimately determine your treatment options.
