I have a lot of fat on my chin not the underneath but the actual chin and it makes it stick out. I am a teenager but I constant comment on why my chin sticks out for so many years. All I have ever wanted is for it to be less and stop sticking out. What’s the best for me or any type of surgery? I do want something permanent.

Your lower jaw is prominent in 3 dimensions; ie not just profile, but width and height. The recontouring of the jaw at the 'back, middle and chin' is necessary if you wish a softer look to the jawline. Typically called a 'V-line'; but modified for your facial balance to avoid a 'too small' look...
The areas of 'dips' relates to the osteotomy/bone cut areas. Your growth pattern can result in this appearance after surgery is the esthetic features of the pattern are not considered. You can augment the jawline and improve the specific contour areas of concern without extensive surgery....
