Epicanthoplasty without asian blepharoplasty? (photos)

Question

Is it possible to get an epicanthoplasty without getting double eyelid surgery? My eyes already have a crease; however, I really dislike the fold I have covering my inner eye corner. I know the folds are not extreme but I would prefer myself without them. If the answer is yes, what is the likelihood that the procedure will result in permanent scarring in the inner corner and would it help that my epicanthic folds are not extreme?

Hello ra12, thank you for your question and photo.  Yes! Medial epicanthoplasty can be done as a stand alone procedure and does not have to be combined with an upper blepharoplasty if you don't need one.  The incision can be quite obvious in the early healing phase, and it does have a higher chance of hypertrophic scarring compared to the upper blepharoplasty incision.  Good surgical design + technique is critical, and the incision has to be tension free, for the incision to heal well.  Good scar management is crucial too - sun avoidance, scar gel, massage etc.  If it becomes hypertrophic, steroid injections would be extremely helpful.  Hope that helps.

Epicanthal fold can be corrected with or without double eyelid surgery by a z-plasty, y-v plasy or mustarde's procedure.

Thank you for sharing your question and photo. Yes, epicanthoplasty can be performed without blepharoplasty, but can leave a scar. By evaluating your photo, it soon are that you do not need either procedure. There is a way to perform an epicanthoplasty of the inner corner of the eyelid without touching the skin over the eyelid by modifying the fold.  There is a chance of scarring, however, this tends to be less noticeable over time once healing is complete.  Non-absorbable sutures lead to less scarring and steroid creams can help as well. Medial epicanthoplsty may be performed independently of double eyelid surgery. The resultant scars which fade with time are a trade off that needs to be accepted.Please use this information when consulting in person with an experienced Board Certified Plastic Surgeon.

I perform Asian Eyelid Surgery every single day of the work week and I wouldn't do it if I were you.  If you felt strongly about creating a parallel crease you could most likely do that and it would have nearly the same look as a medial epicanthoplasty without the scarring.  There is no such thing as an invisible scar, ever, but medial epicanthoplasties have some visible scarring almost always.I'll attach some photos but if you look at the work I've done here none of these patients have had medial epicanthoplasties.Chase Lay, MDAsian eyelid surgery expert

I think you are a good candidate for medial epicanthoplasty only surgery if you understand the risk of scaring in the early part of the healing process. The scars usually heal well but it can take up to many months.

Epicanthoplasty requires multiple cuts in the inner canthus. These will leave a scar.If you can not accept a scar then do not do the surgery
