In my opinion, inflammation is a key factor in hair loss. It is extremely important for you to determine the etiology of your scalp redness. A consultation with a hair expert will include a good history, Woods and scalp exam, blood work and most probably scalp biopsies. I'd suggest you see a dermatologist for a discussion of some more advanced treatments. You'll want to make sure this truly seborrheic dermatitis (which it likely is) but of course other possibilities exists, including a scarring variant of seborrheic dermatitis known as seborrheic folliculitis. The main ones to exclude are psoriasis and other forms of scarring alopecia. For seborrheic dermatitis refractory to treatments there are a few options include itraconazole, clobetasol shampoo and oral retinoids (isotretinoin, low dose). There are prescribed mainly by a dermatologist. Some with seborrheic dermatitis (especially those with light brown/sandy/blond hair) often have persistent redness which is stubborn to treatment. The important thing is differentiating how much hair loss is attributable to seborrheic dermatitis and how much is attributable to the androgenetic alopecia. Minoxidil remains an option in addition to the finasteride you are on. Please speak to your physicians about options if hair loss persists.
