In a tummy tuck the skin of the lower abdomen is removed.  Usually the skin excision involves an ellipse of skin that goes horizontally from hip bone to hip bone and vertically from the belly button to the pubic bone.  If the tattoo is within that area it will be removed with a tummy tuck.  Unfortunately, most tattoos that I have seen in the lower abdominal/groin area are positioned so that only the upper half of the tattoo can be easily removed with a tummy tuck.  Sometimes it is best to leave the lower portion of the tattoo and look laser removal afterwards.  If the tattoo is too low, excision of it with a tummy tuck would lead to an abnormal scar. Without pictures, it is difficult to answer this question well.  If the tattoo is located between the belly button and the pubic area, it should be able to be removed in a full tummy tuck.  An in person consultation with a board-certified plastic surgeon will give you the best answer.

Based on the very low position of your tattoo, it would be capable of being removed with a mini-tummy tuck. (skin cutout below the belly button) It is too low, however to be completely removed by a full tummy tuck as the amount of skin removal to do it is too great to get the wound closed after removal. Your decision is one of getting the best tummy tuck result and leaving much of the tattoo or getting rid of the tattoo but with less than the maximum result from a more limited tummy tuck procedure.
