Scar Tissue Capsule in Chin - Removal and Revision Options? (Photos)

Question

I previously had a chin implant that was later removed, but the scar tissue was left behind by my previous surgeon for an "augmented look." This has created a noticeable bulge in my chin that I'm unhappy with. I'd like to know: Can this scar capsule be permanently removed through an intraoral incision (so there are no external scars)? Is removal alone enough or would you recommend combining it with a chin reduction/shortening for better balance? What kind of improvement can I realistically expect compared to my natural pre-implant chin? Thank you in advance!

I am doing genioplasties 25 yrs ago and I have received cases like yours.

The decision of "leaving the capsule for an augmented look" taken by your surgeon was simply dumb, pointless and erroneous, just happens he wished a swift and hassle-free maneuver, pull out the implant and period.

However, after inserting a chin implant the surgeon, necessarily and due to anatomical reasons, has to dettach the mentalis muscle and its tendon off the surface of the chin/jaw; this is not a major issue if the mentalis muscle finds spontaneous reattachment over the chin implant; however... when the chin implant is removed... the chin pad, the mentalis muscle and the skin hood around them become loose given they are missing their bony insertion, acting droopy and showing a dynamic distortion (when pouting, lower lip incompetence, bulkiness, etc); does this mean the chin implant can't be removed? of course not, but... unless you visually confirm there remains enough mentalis-chin connection... performing a trans-osseous reinsertion of the mentalis is a MUST... otherwise things end like you suffer now.
In short, you need a surgeon with SOLID experience in revision genioplasties and trans-osseous reattachment of the mentalis, with IMO is a very easy maneouver, I don't understand why some cases fail to receive it.

The photos alone are insufficient to provide an accurate assessment. Some of the things that I notice include: deep mentolabial fold, the most prominent point of your chin is too superior, the soft tissue chin pad is thick, and the chin appears a bit wide. Based on your concerns, any aspect of your chin can be changed to improve the overall aesthetics. When an implant is removed, the soft tissue capsule returns to its original form, and there are minimal residual scar effects.

It is very likely that small scar caused by capsule retraction may be reason for this mild deformity? Removal of the scar may be very challenging from intraoral incision but possible. Based on your before and after photos I still believe that a small augmentation with properly selected implant would be meaningful? Good luck.
