Chin tight, ball look to it. Is it worth going back to my same surgeon? when I brought this up to him during a month follow up appointment he said Botox would help the hyperactive mentalis muscle. If I needed a revision with the risks like lip-numbness, smile, reopening gumincision outweigh the revision? Will this get better with time or is a revision needed?

Vast majority of surgeons attending chin deformities stay short of using just chin implants or fillers, a very bad choice.

Some bold and more skilled dare to perform basic sliding genioplasties, not always with optimal results like in your case.

The complete mastering of chin surgery has to include multidimensional genioplasties and the professional has to be able to wisely combine slidings, reductions and augmentations with bone grafting in the same case (if needed or deemed indicated).

I'd bet you have a sliding with the following mistakes:
-the surgeon over increased the vertical heigth in a aesthetic misjudgement, by means of creating a gap between fragments bridged by a titanium plate (get an X ray to confirm this)
-you originally had a overly sharp chin, now at the sliding this surfaces further and becomes more visible, creating a so called (no offence intented) witch's chin, it should have been rounded/widened to prevent this paradoxical undesired effect

I recommend you get a revision with a polivalent genioplasty surgeon.

The procedure is very versatile because the chin can be repositioned in multiple directions to improve the aesthetics and meet your expectations. Usually, the plate and screws are removed, and the chin is repositioned to a more aesthetic position. You are entitled to a satisfactory result.
