I am 3 months post hair transplant op and I have very red and spotty donor area. I think it could be Folliculitis? (photos)

From the photos you have sent it seems that you are experiencing inflammation of the hair follicles.This can occur due to:
formation of ingrown hairsirritation beneath the skin surface as a result of the emerging hair shaftthe extraction sites can erupt in acne when hair damaged during the extraction process become ingrown as they try to regrow
You should take up the matter with the surgeon who treated you and contact the clinic where the operation was carried out as soon as possible.

Your photos aren't too clear but there is something unusual going on- give your surgeon a call and go in for evaluation

You have what appears to be buried grafts from a faulty FUE technique. These are infected buried grafts which will probably require surgical drainage by an expert.
