Rippling despite implant revision (Photo)

I had a double mastectomy with reconstruction I had to go with a smaller size then the tissue expanders went as I had near implant exposure in an area on my right breast. I waited a year for the skin to heal for my surgeon to offer an implant exchange as I had severe rippling. i still have severe rippling after the implant exchange with most cohesive implant available. I tried putting on weight to be eligible for fat grafting but surgeon is wary to do it bc of possible contouring issues. Help?
