You have a very high hairline and your eyebrows are asymmetrical with one being below the level of the orbital rim. I recommend a browlift. This will lower your hairline, decrease the vertical height of the forehead, correct the asymmetry of the eyebrows and lift some of the upper eyelid skin. After this, you may or may not need an upper blepharoplasty.

First, don't be so hard on yourself. I really don't think you "need" to change much of anything. However, the expert use of an appropriately robust HA volumizing filler for cheek augmentation would not only create a more desirable, feminine, oval, heart-shaped face and Ogee Curve, but would also address the hollows under the eyes, since the upper cheeks are a major support for the infraorbital area.
And when you say you had no success with fillers in the past, I would guess that you were likely treated by an inexperienced injector who directly treated your under-eyes, rather than addressing the cheek volume issue that is the primary cause of the hollows.
So, if you are going to do anything like expensive, aggressive surgery that risks scarring and prolonged downtime, you would be wise to first consult with an experienced, board certified cosmetic dermatologist filler expert to explore the above. In the hands of an experienced injector, filler cheek augmentation should take no more than about ten to fifteen minutes to perform, with immediate, on-the-spot results, and little to no significant downtime.
